Flint Hills Community Health Center is a Nonprofit 501(c)(3) Federally Qualified Health Center with Patient-Centered Medical Home status. We provide medical, dental and behavioral health services on a sliding-fee scale. We serve everyone, with or without insurance. To meet your needs, we offer walk-in services, same-day appointments, extended hours, in-house lab, immunizations, prescription assistance, free interpreter services (servicios de intérprete gratis), and free transportation in Lyon County.

Flint Hills Community Health Center has a rich history in our community.

The Lyon County Health Department was founded in 1923. From its humble beginnings, it grew from providing home visits and immunizations to offering more comprehensive public and personal health services.

In 1997, the health department received a grant to become a Federally Qualified Health Center and offer medical, dental and psychiatric services to everyone, including individuals with insurance, Medicare, Medicaid and the uninsured. As an FQHC, its service area included Chase, Greenwood, Lyon, and Osage counties.

By 2002, the FQHC officially became Flint Hills Community Health Center, Inc., and separated from the county, but the health center still contracts with Lyon County to offer immunizations, disease investigations and environmental health services.

In 2013, the health center received a grant to open a new health center location in Eureka. This site provides medical, dental and behavioral health services for the residents of Greenwood and Woodson counties.

Patient-Centered Medical Home
Flint Hills Community Health Center is a Level II Patient-Centered Medical Home, meaning the focus is on you and your family in your care and disease management.

We help form a team that includes you, your family, your primary care provider, nurses, other specialists, dietitians, behavioral health consultants, your case manager and your community health workers. We help you reach your goals by improving communication with the team; help you manage your off-site referrals and appointments; and more!
