Who Qualifies?
Children may qualify for HealthWave if they:
Are Kansas residents
Are U.S. citizens or documented non-U.S. citizens
Are under the age of 19 (if 18, a separate application may be necessary.)
Meet the current income guidelines listed on the application insert.
Some Services Covered Are....
Medical Services - Office visits, regular check-ups, immunizations, hospital services, inpatient and ouptiatient
hospital, lab and x-ray, prescription drugs,eye doctor exams and glasses, hearing services, home health services
and speech, physical and occupational therapy.
Dental Services for Children - Checkups, cleanings, sealants, x-rays and fillings.
Mental Health Services - Inpatient and outpatient mental, behavioral and substance abuse services.
What Does It Cost?
Most families qualify with no out-of-pocket expense.
Some families may have to pay a $20 or $30 monthly premium per family.
If you are 18 or older, you may have co-pays of $1 - $3 for some benefits.
