improving healthcare standards and
educational levels
for diabetic patients.
This collaborative is dedicated
to ensuring that diabetic patients receive the following
exams, screenings, and education:
Instruction and goal setting for
management of diabetes: including educational classes on
nutrition and other components that impact daily management
of the condition.
- Annual Foot Exams
- Annual Retinal Eye Exams
- Routine Dental Examinations
- Annual Cholesterol Screenings
- Semi-annual HgbA1C Screening
National Health Care Collaborative
The Flint Hills Community Health Center participates in
the Health Disparities Collaborative with the Bureau of Primary
Health Care. This collaborative is dedicated to ensuring
that our patients with diabetes, depression, and cardiovascular
disease receive the quality care and testing necessary to
treat their disease. These may include:
- Instruction and goal setting for management of diabetes,
depression, and hypertension
- Educational cooking classes on nutrition for diabetes
- Depression support group
- Annual foot exams, retinal eye exam referrals, routine
dental exam, and laboratory examinations for diabetes care
- Behavioral Health Consultant
Services now offered include a Chronic Case Manager and
Promotara de Salud, also known as a Community Lay Educator.
These positions work in the community, seeking to reach clients
in need of education for diabetes, cardiovascular disease,
and depression. Educational materials are also available
for these chronic diseases.
